
About This Role
Alright, let's talk about this role. It's for a Behavioral Health Specialist with Oak Street Health in Winston Salem, NC. You'd be embedded in their primary care clinics, working directly with patients and their care teams. The core of the job is managing behavioral health needs; screening, short term therapy, care coordination; to support the whole person.
What you'd do
You're the go to person for behavioral health across a few clinics. You'll work hand in hand with primary care providers to figure out which patients could use your support.
- Screen and assess for common mental health and substance use issues.
- Create care plans and handle referrals, whether to our internal psychiatric consultant or outside specialists.
- The therapy side is short term and evidence based. Think Motivational Interviewing, Behavioral Activation, Cognitive Behavioral techniques.
- A big part of the job is coordinating. You'll connect with our psychiatric consultant for warm handoffs and to collaborate on treatment plans.
- Building solid relationships with the clinic teams is crucial. You'll keep everyone updated on any changes to a patient's behavioral health plan.
- All your patient notes and counseling sessions need to be documented accurately and promptly in our electronic medical record.
- There's also a chance to help shape the program itself through workshops, surveys, and feedback sessions.
The schedule is full time at 40 hours a week. And there's potential for some remote work days down the line, depending on how things are going with program metrics.
What you need to bring
A few things are non negotiable here.
- You must have a Master’s Degree from an accredited school in Social Work, Counseling, Marriage and Family Therapy, or a similar mental health field.
- The highest clinical level of license (or local equivalent) is required. We're talking LCSW, LCMHC, LMFT; that tier.
- Experience screening for common mental health or substance use disorders is a must.
- Same goes for experience with assessment and treatment planning for those conditions.
- You should be familiar with brief, structured intervention techniques like the ones mentioned above.
- Authorization to work in the US is required.
A few things that would really help
- We'd love it if you were proficient; not just familiar; with those short term therapy methods (Motivational Interviewing, etc.).
- A working knowledge of differential diagnoses for common behavioral health conditions would be great too.
The pay & benefits picture
The typical pay range for this position is between $54,095 and $129,615 per year. Where you land in that range depends on your experience, education, location, and other relevant factors.
