Alright, let's talk about this RN Case Manager role. It's a bit of a hybrid position; you're office based but the work is all about connecting with and supporting a specific community. The team is small and collaborative, focused on serving predominantly Spanish speaking women who are 40 and older in Baltimore City.
What You'll Be Doing
The core of this job is telephonic patient engagement and clinical assessment. You'll be the point person for care coordination, using your clinical judgment to figure out what people need and how to get it for them. It's about making sure care is continuous and appropriate.
- You manage a caseload from admission all the way through discharge, and sometimes follow up post discharge with calls to check that plans are being followed.
- A big part of your day is communication. You're talking daily with direct caregivers, the case management team, patients, families, and outside resources to coordinate everything.
- You'll perform comprehensive assessments to identify patient specific needs related to their diagnosis, treatment, and especially their discharge planning.
- Collaborating with the whole healthcare team; including utilization review; to develop and adjust the plan of care is central to the role.
- You'll evaluate how patients are responding to their care plans and document outcomes, making recommendations for changes when needed.
- The job involves identifying potential delays in care or barriers to a smooth transition out of the hospital and communicating those issues appropriately.
- Sometimes you delegate specific needs, like connections to community resources or social work support, to other specialized team members like community health advocates or social workers.
- A current RN license is mandatory.
- You need several years of recent acute care nursing experience under your belt.
- Strong clinical assessment skills are non negotiable; you're making triage decisions and identifying needs over the phone.
- Fluency in Spanish is required because that's the primary language of the population we serve.
- A deep understanding of discharge planning, Medicare/Medicaid reimbursement basics, and regulatory requirements for different levels of care is expected.
- The role requires some travel within Baltimore City for meetings or patient visits.
- The fixed annual salary is $86,000. It’s eligible for annual cost of living increases based on the grant funding cycle.
