The Role

Alamo Hospice, part of the Addus family of companies, is hiring an RN Case Manager. You'll be based out of Waco, TX and cover Waco and the surrounding counties. This is a full time position, Monday through Friday, 8a to 5p, with an on call rotation of 1 2 days per week, occasional weekend backup, and rotating backup shifts.

What You'll Actually Do Day to Day

You'll take primary responsibility for a caseload of patients and their families. This means you're the one handling the full nursing process: assessment, determining eligibility, admission processing, planning, implementation, and evaluation. You'll gather data from the patient's physical, psychological, social, and spiritual factors to understand what might impact their needs. When something comes up that another team member should handle, you'll kick off that communication.

  • Communicate with the attending physician, the interdisciplinary team, and outside agencies to coordinate the best care possible.
  • Make regular visits to re assess patient needs and update the plan of care.
  • Handle clinical pronouncement of death and provide support to the family.
  • Maintain accurate patient records, charts, progress notes, and reports, and prepare care plans on time.
  • Supervise and document the delivery of care by the Home Health Aide, following federal, state, and company guidelines.

What You Need to Bring

You need to be a graduate from an accredited registered nursing program. You must have a current RN license in the state where you'll practice. Current CPR and First Aid Certification are required. You should know your way around quality assessment and assurance procedures. And you need a valid driver's license with proof of insurance.
