Case Manager RN

Hey there. I'm a recruiter at Ascension, and I've been doing this for over a decade. I'm looking for an RN to join our Case Management team at Saint Thomas Midtown in Tulsa, OK on a PRN, day shift schedule.

This isn't just chart review. You're essentially the architect for a patient's entire hospital journey. You'll coordinate their medical, social, and even spiritual needs from admission until they safely transition home or to another care setting. It's about identifying real world barriers; like no insurance, unreliable transportation, or a lack of support at home; and finding practical solutions to prevent them from bouncing back here. You're also the crucial link between our clinical team and insurance payers, making sure the documentation supports the level of care we're providing.

What you'll need

• A Registered Nurse license from the Tennessee Board of Nursing. If you have a multi state compact license from your home state, that works too; it just needs to be in hand before you start.
• A BLS Provider certification. We prefer it comes from the American Heart Association or the American Red Cross.
• Your nursing diploma from an accredited school or college, or your required professional licensure at the time of hire.

It'd be great if you also had a Case Manager certification from either the Commission for Case Manager Certification (CCMC) or the American Case Management Association (ACMA). But if not, we can talk about it.

The details

This is a PRN position working day shifts. Your specific schedule and hours will be variable based on department needs.
