About This Role

What you'll actually do

This isn't a desk job. You'll be embedded in nursing facilities in Trenton, NJ two to three days a week or more, depending on what members need and how acute their situations are. Your real focus is getting people out of those facilities and back home safely, then making sure they stay there.

You manage incoming referrals for nursing facility transitions. You assess whether someone is ready to return to their community. You facilitate Interdisciplinary Team meetings and build a transition plan that's actually about what that person wants, not just what's on paper. There's no traditional caseload here. The goal is three successful transitions per month.

But it doesn't end at discharge. Within 10 business days after someone goes home, you meet them face to face to complete a NJ Choice Change in Condition assessment. You also check in within 7 business days for a Significant Change Visit. You're making sure the home environment is set up right, that the ramp or DME is installed, that they're not about to fall or end up back in the ER.

You also accompany members to medical appointments when it makes sense. You coordinate transportation. You schedule appointments and follow up to make sure people actually go. You educate members on fall prevention and medication adherence. And you attend workgroup meetings where you help the whole team get better at transitions.

Once a week, you call the facility's business office to check on census and coordinate with social services and case management. You also work with health plan case managers to spot high risk members before they lapse. The goal is to keep people in the community for at least six months after they transition.

Where you'll be working

You need to live in Northern New Jersey. Specifically, we're looking in Trenton, NJ for someone residing in one of these counties: Warren, Sussex, Morris, Passaic, Bergen, Essex, or Hudson. You'll travel up to 75% of the time in that coverage area. You need reliable transportation. Mileage is reimbursed under our standard expense policy.

Your license and experience

  • Active, unrestricted RN license in good standing in New Jersey
  • At least 2 years of clinical experience
  • Experience in managed care, discharge coordination, transition of care, home health, case management, or Medicaid
  • Associate degree in nursing is required; a BSN is preferred
  • 2 or more years of clinical nursing in home health, case management, or discharge planning is preferred
  • You need to be comfortable navigating multiple computer systems

Schedule and pay

This is a full time position at 40 hours per week. The pay range for this role is $46.43 to $82.90 per hour. Your actual pay will depend on your experience, skills, and where you live within that range.
