About This Role

What this job is

This is a full time role for an RN who works inside the hospital. You're the bridge between the hospital's care teams and our hospice services. When a patient in the hospital needs to transition to hospice, you're the one who makes it happen smoothly, whether they stay in the hospital or go home.

The schedule is Monday through Friday, 10am to 7pm. There's some limited on call duty. The pay range is $90,000 to $110,000 a year, depending on your experience.

What you'll do here

Your main job is building real relationships and being the expert resource for hospice care. You'll be in the hospital every day, talking with people.

  • Build and keep trusted relationships with doctors, hospital staff, and care teams. This happens through daily, in person collaboration.
  • You're the primary clinical resource for hospice education. You'll explain eligibility and whether it's the right fit.
  • Conduct patient assessments and coordinate the entire admission process into our care.
  • Partner closely with our Admissions, Clinical, and Care Teams to make sure every transition is seamless.
  • Advocate for patients and their families during goals of care conversations.
  • Participate in interdisciplinary planning meetings, training sessions, and community outreach.
  • Uphold high standards for ethics, quality of care, and your own continuous learning.
  • There are clear expectations for response times, referral coordination, and communication that you'll need to meet.

The background we're looking for

We need an RN who knows how to navigate a hospital environment independently and can talk shop with physicians. A little business savvy goes a long way here.

  • A current, active RN license is required.
  • You need at least one year of experience as an RN.
  • Knowledge of home health regulations and Medicare requirements is important. So is understanding care plan development.
  • A valid driver's license and an insured vehicle are required.
  • If you've got medical sales or clinical outreach experience, that's a definite plus.
