What You'd Be Doing
You're the guide for patients and their families through the whole care process. That means connecting them with resources, building care plans that actually fit their lives, and making sure everyone on the team is on the same page. Your days are about coordination and clear communication to keep things moving smoothly from admission to discharge.
- Walk patients and families through how things work: the rehab philosophy, what their Medicare or insurance covers, discharge criteria, and their rights.
- Get patients, families, and caregivers involved by coordinating education for treatment planning, goal talks, and family meetings.
- Put together individualized treatment plans that focus on a patient's strengths and personal recovery goals.
- Complete psychosocial assessments and discharge planning evaluations. You'll document everything clearly and share what you find with the rest of the care team.
- Work side by side with the Rehab Program Director, Facility Case Management Director, nurses, physicians, and department leaders.
The Stuff You Need to Bring
We need someone who knows their way around a hospital setting. You've got to be a licensed RN here in the state. An Associate Degree in Nursing or a Nursing Diploma is required; if you have your Bachelor’s in Nursing, that's even better.
- You need either 2+ years of experience specifically in case management OR 3+ years of experience in clinical nursing.
- A current Registered Nurse license for our state is mandatory.
- If you've used InterQual software before or have a Certification in Case Management, we'd really like to see that.
The role demands someone who can build good working relationships quickly. You'll need to communicate clearly whether you're talking face to face, writing notes, or sending an electronic message. We're looking for analytical thinking skills; the kind where you can make proactive decisions and negotiate when you need to.
