
About This Role
The Role
Kaiser Permanente needs a Registered Nurse to provide skilled nursing care to patients in their homes through the Seattle, WA hospice, palliative care, and home health programs. You'll work on an interdisciplinary team, handling patient assessment, creating care plans, and providing education on pain and symptom management. The goal is to help patients and their families adjust to changes in health status. You'll also plan for when care returns to a primary care provider.
What You'll Do Day to Day
- Collaborate with other team members and the patient's family to plan and deliver care.
- Record accurate data in the electronic medical record on time, following documentation guidelines.
- Follow KFHPW standards for infection control, safety, hazardous waste, universal precautions, and CPR.
- Provide nursing interventions that require specialized skill, and start appropriate preventive and rehabilitative care.
- Carry out the care plan: instruct patients and caregivers on disease management and adjusting to illness; supervise LPNs and HHAs in the home; perform treatments; teach self care and home safety; manage pain and symptoms at home.
- Perform skilled procedures like injections, dressing changes, catheter insertion and care, colostomy and tracheotomy care, central line IV care, and blood draws.
- Identify what patients need to learn and teach them according to the care plan.
- Manage cases as part of the interdisciplinary team.
- Figure out your own learning needs, including those specific to different age groups, and take part in educational activities to meet them.
- Participate in staff orientation, self and team performance reviews, and activities that improve care quality and save costs.
- Follow KFHPW personnel policies and maintain professional work habits. Keep your appearance consistent with the dress code.
- Assess whether a patient needs care and communicate with other health care providers on time.
- Collect data during visits over the whole course of care.
- Develop and start a care plan based on your assessment.
- Talk with medical doctors to set up the care plan.
- Evaluate how the patient responds to care through timely assessment and observation.
- Change the care plan based on how the patient and care systems respond.
What You Need
- At least two years of experience as an RN in medical surgical, home health, or hospice. If you're a current KP Coalition employee, that requirement is waived per the National Agreement.
- An Associate's degree in Nursing from an accredited program.
- A high school diploma or GED.
- A Washington State Registered Nurse License at hire, or a Compact License at hire.
- A Washington State Driver's License at hire.
- Basic Life Support certification at hire.
- Proof of automobile insurance.
- Experience doing care planning within a community setting (continuum of care).
- The ability to work on your own.
- The ability to transfer or position patients as needed.
- Basic laptop skills, including MS Office Suite.
- Customer service skills.
Nice to Have
- Experience with the Horizon for home care system.
- A Bachelor's degree in Nursing or a related health care field.
