Alright, let's talk about this job. It's not a bedside nursing role. You'd be the bridge; the person who makes sure someone who needs hospice care can actually get it, smoothly and with dignity. You're connecting patients and families to our services at a time when they're most vulnerable, and you're the trusted point of contact for every hospital discharge planner, case manager, and physician in your area. What you'll do
Your main focus is handling referrals and guiding admissions. That means you're out there, visible.
- You get the call or the referral from hospitals, home health agencies, doctors' offices, nursing facilities; you name it. You respond.
- Figuring out if someone is eligible for hospice is on you. You'll review medical records, do clinical evaluations, and have frank conversations with physicians and families.
- A huge part of this is education. You explain what hospice care really is; how it works, why it matters; to patients and families who are often scared or confused. Clarity and compassion are your tools here.
- All the intake paperwork and coordination falls to you, making sure everything meets clinical, legal, and insurance requirements.
- You are the liaison. You make sure the handoff from a referral source to our care team isn't just a paperwork shuffle; it's a seamless transition for the patient.
- Expect to be in hospitals and facilities daily. Your face needs to be known by discharge planners and case managers.
- This is also a relationship business. You'll maintain the connections we have with referral partners and look for new opportunities to raise awareness about hospice services in the community.
About you
We need someone who already speaks the language of hospice care.
- You must be a Licensed Nurse with three to five years of hands on hospice experience. That's required.
- A BSN degree with five or more years in hospice? That's definitely preferred.
- The minimum is three years in hospice. If you've worked as a discharge planner or case manager before, that's a big plus.
- Hold a current RN license for the state where you'd be working.
- You need CPR certification, a valid driver’s license, and auto insurance.

- A deep understanding of hospice care and what patients and families truly need during this time is non negotiable.
- Knowing your way around community resources and local referral networks helps immensely.
- Your patient assessment skills have to be sharp; so does your documentation.
- Sensitivity is key; you're working with people from all walks of life during intensely personal moments.
- The role demands flexibility. Priorities shift quickly; you have to manage that without losing your cool.
- You'll work with an entire interdisciplinary team. Professional maturity and knowing how to collaborate are essential.

The salary range for this position is $114k - $124k annually. A candidate's final pay within that range can depend on things like their specific location within San Bernardino, CA, their experience level, education, skillset, licenses held, and internal equity.
