
About This Role
Alright, let's talk about this role. It's not your typical bedside job. You're the bridge between the hospital and home, making sure that transition doesn't just happen, but happens well. If you're the type of clinician who sees the bigger picture in a patient's journey and wants to own a critical piece of it, this is for you.
What You'll Do
You're the point person for getting patients safely from an acute care setting back to their homes. That means you're in hospitals, at bedsides, and on the phone, connecting all the dots so nothing gets missed.
- You'll go into hospitals to conduct bedside assessments within 24 hours of getting a referral.
- Your job is to build a transition plan that actually works for the patient, using solid clinical guidelines.
- Talk to everyone: patients, their families, case managers, doctors. You're gathering all the intel needed for a safe discharge.
- Spotting high risk medical or social needs is a big part of this; you'll flag those for the care team.
- You make sure a follow up with their primary care doctor is scheduled within three days after they leave the hospital.
- A check in call happens within 48 hours of discharge. You'll document that coordination note in our system, HCHB.
- A lot of this work is about preventing avoidable return trips to the hospital through clear communication and smart planning.
The Nitty Gritty Details
There's coordination work and there's paperwork. This role has both. You'll be documenting to support our clinicians who admit patients for home health and making sure all the pieces move forward on time.
- Documenting your coordination notes in HCHB is key to setting up the home health team for success.
- You'll handle specific tasks and workflows in our electronic medical record system that are built for this role.
- Sometimes you'll take verbal orders from licensed practitioners; entering those into HCHB and getting physician approval is part of the process.
- Following up on pending referrals keeps everything moving so we can admit patients without delay.
Working With Others
You're never on an island here. This job is pure collaboration across our agency staff, contractors, patients, and all our referral partners.
- Clear communication with every provider on a patient's care plan isn't just nice; it's essential.
- You'll help teach patients and caregivers how to use our VitalCaring Connection platform for virtual and phone based care support.
- Sitting in on case conferences with other healthcare team members is standard practice; you come prepared to contribute.
The Professional Stuff
We expect you to keep your skills sharp and be a reliable part of the team. It’s pretty straightforward:
- Keep up with all mandatory continuing education requirements. Show up ready to communicate effectively with everyone you work with. Attend any in service training sessions our agency sponsors. And yes, there will be other duties as assigned; that’s just how it goes sometimes.
The Must Haves (No Exceptions)
- A graduate from an accredited nursing program (as an RN or LVN/LPN) or an accredited Physical Therapy program (as a PT). An active RN, LVN/LPN, or PT license for San Antonio, TX; plus a valid driver’s license. Depending on your license type, you might need to complete some HHS Computer Based Training. At least two years of clinical experience under your belt.
