Case Manager RN

The Role

This is a Case Manager RN position at Methodist Hospital Specialty and Transplant, working a full time weekends schedule. You'd pick either Friday/Saturday/Sunday or Saturday/Sunday/Monday ; three 12 hour shifts each week.

What You'll Actually Do

Walk patients and their families through every phase of their care. That means explaining how the rehab program works, what Medicare and insurance covers, discharge criteria, and patient rights. You'll coordinate education for families and caregivers so they know how to participate in treatment planning, goal discussions, and family conferences.

• Build individualized treatment plans based on each patient's strengths, needs, and personal recovery goals
• Complete psychosocial assessments and discharge planning evaluations, then document everything clearly and communicate it to the care team
• Work closely with the Rehab Program Director, Facility Case Management Director, and the interdisciplinary team to keep care coordinated and centered on the patient
• Promote HCA Healthcare's values of compassion, respect, and excellence in every interaction with patients and families

What You Need

• A diploma, associates degree, or BSN from an accredited nursing program ; a Bachelor of Science in Nursing is preferred but not required
• A current RN license to practice in Texas or an active Multi State RN Compact License (if you have the compact license you'll need to get your Texas RN license within 90 days of hiring)
• At least 3 years of RN experience under your belt
• Experience in utilization review, resource management, discharge planning or case management is preferred but not mandatory

Full time weekends. Three 12 hour shifts per week. Your choice between Friday/Saturday/Sunday or Saturday/Sunday/Monday.
