Alright, let's talk about this PRN Social Worker role with HCA Virginia Healthcare at Home in Richmond, VA. It's not a regular full time gig; you're on an as needed basis. That means flexibility, but it also means you need to be ready to go when we call.

What the job is

You'd be joining our home care team. Your main focus is implementing the standards of care for medical social work services for our patients. You'll set the goals and priorities for social services here, making sure everything we do meets quality standards and fits with our mission.

A big part of your day is assessing the psychosocial status of patients and their families or caregivers, specifically related to the patient's terminal illness and their living situation. You're not doing this in a vacuum; you'll communicate what you find to the registered nurse and the rest of the interdisciplinary care team.

• If a patient's home isn't a safe option, you provide that assessment and help figure out another plan.
• You act as the go between for patients, families, and community agencies.
• To make sure home care services run smoothly, you directly oversee and coordinate care with other clinical service providers.
• You help develop each patient's individualized plan of care with them and their family.
• Attending scheduled interdisciplinary group meetings is part of it, where you'll help the team understand how psychosocial stresses affect the symptoms of the terminal illness.

What you need to bring

We need someone who's already licensed and has a solid educational foundation. Here are the non negotiables:

• A Master’s Degree in Social Work. We can also consider degrees in other human services fields like sociology, special education, gerontology, rehabilitation counseling, or psychology.
• A state Masters of Social Work (LMSW) license is required.
• You've got to have previous experience working directly with individuals in a healthcare setting.
• Reliable transportation is a must. So is proof of valid automobile liability insurance and a valid driver’s license.
