Social Worker

What you'll actually do

You're a Social Worker who wants to work directly with mentally ill patients in the community, helping people who are going through tough times. This role is all about getting out there and making a difference.

You'll screen telephone referrals from people who need help. You perform psychosocial evaluations and assessments of mental health needs for patients and their families, using your professional knowledge, observation skills, and interviewing techniques. You develop short term service plans in collaboration with the Program team. Counseling, care management, and appropriate referrals for long term or supplemental treatment are all part of what you do.

You prepare case histories and maintain records according to the Program's record keeping system. Sometimes you need to encourage resistant clients to accept mental health services by working with them or their family members and friends. You participate in interdisciplinary team meetings and case conferences. You make referrals to long term mental health services, social services providers, or residential facilities.

Coordination is key ; you follow up on linkages between clients and other agencies to ensure continuity of care. You also liaise with community agencies and provide consultation when needed. Outreach is part of your job: you go into the community to find mentally ill individuals who are experiencing difficulties or are at risk, providing intervention in their homes.

You serve as a resource person for the Program and other Agency components regarding psychiatric nursing. You help develop and implement in service training alongside the team. You assist the Program Coordinator with overall service functioning and step into that role when they're absent. If asked, you participate in community programs and education.

You contribute to clinical and administrative policies, procedures, and manuals as needed. Depending on your experience level, you may supervise Mental Health Technicians, graduate Social Work students, or junior staff Social Workers. For Senior Citizens accepted into the program, you monitor their mental status.

A note for Mobile Crisis Float Team applicants: If you're applying for that specific role (code 4711), you'll assist other Mobile Crisis Agencies across all five boroughs with 24 hour notice. That means being part of a flexible response team ready for crisis intervention anywhere in NYC ; it takes adaptability and solid coordination skills.

You'll also pitch in on special projects and any other duties that come up.

What you need

• A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education ; plus certification or licensing by the New York State Education Department to practice social work here
• A current registration as a Licensed Master Social Worker (LMSW) in New York State
• At least one year of experience as a Social Worker in a healthcare setting
• A valid driver's license might be required depending on operational or regional needs
• Bilingual skills could be required based on what we need operationally

The pay

$33.85 - $42.33 per hour.
