Hey, it's me again. I'm putting out the call for a Per Diem LPN to work with us at Far Rockaway Center in Queens, NY. We're a 100 bed rehab and skilled nursing facility, and honestly, our size is our secret weapon. It lets us keep things personal. We know everyone's name, and we're all committed to the same thing: making sure our residents have the highest quality of life while we protect their dignity and independence every single day.

The Work You'll Do

This is hands on nursing care. You're the one they'll rely on from admission through their daily routine.

• You’ll handle new resident admissions, collecting all the necessary information to get them settled.
• A big part of your day is monitoring and recording health details; vitals, temperatures, food and liquid intake, all of it.
• Administering medications and injections as prescribed is central to the role.
• We need someone skilled in wound care, ready to treat and dress wounds or bedsores when needed.
• You’ll help residents with dressing and personal hygiene tasks.
• There are times you’ll provide supervision to our team of Certified Nursing Assistants (CNAs).

What You Need to Bring

• A valid New York State LPN license is non negotiable.
• That license needs to be in good standing with the State Registry.
• And you've got to be someone who can plug into a team. We succeed together here.

The Details & The Offer

Here are the hard numbers and what comes with the job. It's per diem work, so your schedule has flexibility based on our needs and your availability.

• The base pay rate is $33.85 per hour.
• Pick up evening or night shifts? That rate gets an extra 10% shift differential on top of the base.
• We're also offering a $5,000 sign on bonus right now.
