Community Health Worker

What You'll Do

You'll work as part of an interdisciplinary care team inside a NORC (Naturally Occurring Retirement Community) or senior living community. The goal is simple: help older adults age safely in place by coordinating health and social services.

Conduct client assessments to figure out health needs. That includes taking vital signs when necessary. You'll document every assessment, intervention, and outcome in clients' medical records ; accurately, every time.

When there's a medical emergency, you provide care until EMS arrives and then you coordinate with them directly. You'll also develop and implement individualized nursing care plans for each client.

Health promotion is a big part of this role. You'll educate clients on disease prevention and health maintenance. And you'll help prepare statistical reports for funding sources ; things like the number of health assessments completed and referrals made to primary care or specialists.

Care coordination and advocacy matter here too. You'll facilitate referrals and linkages to the right healthcare services based on what each client needs.

What You'll Need

• License and current registration to practice as a Registered Professional Nurse in the State of New York
• CPR/BLS certification
• Associate's degree in nursing from an approved program
• Bachelor's degree in nursing preferred
• Minimum of two years' clinical home care experience

Schedule and Pay

The pay range for this role is USD $40.95 to $51.19 per hour.
