We're looking for a Medical Social Worker to join our hospice team in Pittsburgh, PA. You won't be just another employee here; you'll be a crucial part of a small team providing care that honors the whole person; mind, body, and spirit. It's challenging, personal, and deeply meaningful.
What you'll do
Your focus is on guiding patients and their families through this part of life's journey. You're the advocate, the counselor, and the resource navigator all in one.
- Provide direct counseling and psychosocial support to patients.
- Step in to help with crisis intervention when it's needed.
- Work with families on community resource planning and how to access those services.
- Help everyone navigate the ongoing healthcare process.
- Handle important paperwork: reviewing powers of attorney, assisting with Medicare applications, and keeping documentation current.
- Collaborate closely with the interdisciplinary care team on planning.
- Advocate for your patients within the goals of hospice care.
What you need to bring
A specific background is required for this role. Here are the essentials:
- A Bachelor of Science in Social Work or a comparable undergraduate degree is required. A Master of Social Work is preferred.
- A current, unrestricted license to practice as a Licensed Social Worker (LSW) in our state is required. An LISW license is preferred.
- You must have at least one year of professional hospice experience. If you have three or more years, that's even better.
- Your experience should include conducting psychosocial assessments, grief counseling, and advance care planning discussions.
- A reliable car for getting to and from patient visits is non negotiable.
