
About This Role
What you'll do
This is a role where you're a clinician, an educator, and a resource for dialysis patients. The focus is on in center hemodialysis, but you'll also need knowledge of home therapies ; home hemodialysis and peritoneal dialysis. Acute dialysis coverage is part of it too. You're working as a member of an interdisciplinary team, using the nursing process to manage patient care.
You'll develop into an expert on in center hemodialysis while keeping up with home hemodialysis and peritoneal therapies. Every shift, you assess patients through interviews, observation, and physical exams to identify actual problems and outcomes. You coordinate their care from start to finish. You act as a consultant on dialysis issues, both inside the hospital and for outside agencies or groups. You consult with patients, families, community agencies, and colleagues from other disciplines.
You use the interventions from the care plan to hit expected patient outcomes. And you systematically evaluate and document progress toward those outcomes throughout the shift. You're accountable for your own practice ; seeking out resources and working on your own development. You share what you know through informal teaching when others need to learn. You take a practical approach to moving and handling patients. You know how to get the right lab specimens. You do bedside lab testing and quality control, following provider orders and hospital lab policies. You document everything to meet service expectations. You supervise Patient Care Technicians and clerical staff. You show clinical competence with complex patients through expert assessment, manual and technical skills, and critical thinking. You make sure all safety checks and daily checklists are done on every machine, including the Main Reverse Osmosis Machine. You operate all related equipment safely, handle minor troubleshooting, and use supplies efficiently to avoid waste.
Work environment and culture
You're coordinating patient care across the interdisciplinary team, collaborating with team members, and pulling in extra resources when needed. You analyze clinical situations systematically and act to get resources or fix problems so you meet or beat expected outcomes. You own problems and stick with them until they're resolved the best way possible. You prioritize your work based on changes in patient condition, changes in assignment, and unexpected interruptions. You identify and use team resources to solve problems or needs. You advocate with physicians on the patient's behalf when needed. You communicate in a way that fits the situation ; giving and accepting constructive feedback from peers. And when the opportunity comes up, you do bedside critical thinking exercises with the nursing team.
Patient education
You figure out what teaching strategies work for each patient and their family, based on their age and needs. You adjust those strategies based on the patient's response, their readiness to learn, and their level of understanding. You educate patients, observe their skills, and document their competency. You take cultural, age, gender, and developmental factors into account when teaching about procedures, illness, or the hospital experience. You keep patients safe by following policies and procedures, and by sticking to National Patient Safety Goals. You use a practical approach to moving and handling patients. And you bring in the right hospital resources to provide specialized education.
What you need
- A College Diploma in Nursing.
- A license as a Registered Professional Nurse (RN) from the Illinois Department of Financial and Professional Regulation (IDFPR).
- Basic Life Support (BLS) certification through the American Heart Association (AHA), completed within 30 days of starting.
