The Role
This is a Per Diem Home Health RN position with Mercy in Oklahoma City, OK. You'd be providing day to day patient care in the home setting. It's not a desk job; you'll be moving between patients, managing different acuity levels, and handling the full cycle of home care from admission through discharge.
What You'll Actually Do
- Handle case management for multiple patients at once, each with their own needs and severity levels
- Conduct admissions to home care services and post hospitalization assessments
- Perform recertifications and discharges as patients' conditions change
- Teach patients about their conditions and self care routines
- Manage dressing changes and IV therapy administration
- Provide access site care and general skilled nursing following policy and procedure
- Keep thorough documentation using the Epic computer system ; this isn't optional, it's how the whole team stays coordinated
- Assist with other needs that pop up in the home to keep patients safe: things like coordinating lifeline services, arranging Meals on Wheels delivery, or pointing families toward public programs they qualify for
- Work visits on an as needed basis following all department policies
What You Need to Bring
Required:
- A diploma from an accredited nursing school (associate's or bachelor's)
- A current state RN license in good standing
- A valid driver's license ; you'll be driving between patient homes so this one matters
- A current CPR certification card before you start treating patients on your own schedule?
