About This Role

Alright, let's talk about this LPN role. It's not your usual clinic gig. You're out in the neighborhoods of New York, NY, going to where people live. The whole point is to keep folks comfortable and out of the hospital by managing their care right in their own homes. You'll be the eyes and ears for our RNs, building a real connection with patients one on one.

What you'll do

Your day is about adapting. You're visiting patients in their homes or care facilities, so good time management is everything for handling your weekly caseload.

  • Monitor patient progress, track health changes, and report everything back to your supervising RN. This means detailed observations, conversations, and providing physical and psychosocial care within the LPN scope.
  • The plan of care isn't static. You'll constantly evaluate what the patient needs and adjust your nursing and care management approach accordingly.
  • A lot of this job is coordination. You're communicating with our VNS Health interdisciplinary team, other providers, and community resources to make sure care is delivered well and without hiccups.
  • Continuity is key. You'll coordinate supplies, provide education to patients and families, and use empathy to make sure nothing falls through the cracks.
  • We give you tech to stay organized; a tablet, phone, and systems to keep electronic medical records accurate and up to date while you're in the community.
  • You'll provide education and support to help patients and their families understand end of life treatment options.

What we need from you

We're looking for someone who's ready for a different kind of nursing pace.

  • A current New York State LPN license and registration is required.
  • A high school diploma or completion of an LPN program is required.
  • At least one year of experience working as an LPN is preferred.
  • A valid driver's license might be needed; it depends on operational requirements in your area.
  • Bilingual skills could be required based on what our teams in New York, NY need.
  • If you've got home care or hospice experience, that's a definite plus.

A couple specific notes

For our Community Mental Health Services (CMHS) teams: experience working in mental health or home care is strongly preferred.For our Hospice roles: You'd be hired primarily for one area; like Home Care visits, shifts at a Hospice Residence, or Crisis Care visits. But you'll get cross trained in all three hospice service areas and will pitch in across them as needed.

The schedule & pay

The hiring pay range for this hourly position is $38.23 to $40.00 per hour.There's also a per diem per visit version of this job. For that role, the pay range is $50.68 to $54.07 per visit.
