The Role
This is a care management position working within the Wellness case management team at VNS Health in New York, NY. It's built around clients with complex psychiatric and medical conditions who have been deemed temporarily unable to work. You'll coordinate services across multiple providers and make sure these clients get linked to both physical and behavioral health services. The supervision here is close, so you won't be out on an island.
What You'll Do Day to Day
- Review completed medical and mental health assessments from the ResCARE clinical team when starting wellness plans for clients on the temporarily unable to work track.
- Confirm each client's acuity level and shape their service plan accordingly.
- Build and monitor wellness plans for clients whose untreated or unstable medical or mental health issues are hurting their employability. You'll coordinate a written plan with input from the client, their family, and any other providers involved.
- Manage a caseload from wellness initiation all the way through completion. That means meeting with assigned clients, tracking their progress, monitoring compliance with the plan, and keeping detailed case notes that are accurate and timely.
- Create an inventory of community resources that match what your clients need. Get familiar with local service providers so you can help remove barriers like transportation or childcare issues.
- Link clients to ongoing service providers, coordinate with those providers, refer where needed, and follow up to ensure compliance and timely completion of wellness plans.
- Work alongside teammates to conduct outreach through phone calls, emails, texts, and field visits when clients miss steps in their wellness plan or stop engaging altogether.
- Advocate for your clients. Educate them and their families on what entitlements or community services they're eligible for. Arrange escorts or transportation to appointments when necessary.
- Sit in on initial training sessions plus ongoing ones to keep your knowledge of serious physical ailments current (as defined by HRA). Collaborate with the wellness health team on psycho educational plans tied to medication adherence.
A Few More Responsibilities
- Keep client case records updated in both the WeCARE wellness care management system and HRA platforms. Coordinate electronic communication across all provider databases as needed.
- Maintain documentation that matches agency standards, regulatory requirements, and wellness care management policies.
- Participate in case conferences with your supervisor along with staff meetings and discharge planning sessions. These help determine whether a client needs a different level of care or is ready for discharge entirely.
- Sit on special projects now and then plus any other duties that land on your plate.
What You Need
- A bachelor's degree in human services or a related field; a master's program in human services is preferred but not required
- A valid driver's license might be required based on operational needs for this New York, NY role
- A minimum of two years providing direct services to seriously mentally ill patients or clients ; this is not optional, it's mandatory
