About This Role

Alright, let's talk about this job. I'm looking for a Physical Therapist Assistant to join the TriStar Healthcare at Home team in Nashville, TN. You'd be working with patients in their homes, helping them get back on their feet after an illness or injury. It's a full time position.

What you'd be doing

You're the one who turns the care plan into action. You'll work directly with patients under the supervision of a Physical Therapist.

  • Your main focus is delivering therapeutic exercises and functional training. The goal is to help people regain their independence and mobility.
  • You'll apply different physical therapy techniques and modalities to manage pain and restore function.
  • A big part of the job is monitoring how patients are progressing. You need to document their responses carefully to make sure the care is both safe and effective.
  • Teaching is key. You'll show patients and their families how to use any assistive devices and guide them through their home exercise programs.
  • You won't be on an island. You're part of a team that includes PTs, nurses, aides, and social workers, all pulling together to support the patient.

The basics you need

This isn't a "learn as you go" role. There are specific credentials we need from day one.

  • You must be a Licensed Physical Therapist Assistant (LPTA).
  • A valid Driver's License is non negotiable; you're going to patients' homes.
  • The required education is an Associate Degree.

The schedule & support

This is a field role where you manage your own schedule within your assigned caseload. They structure it so your caseload lets you focus on your patients without being spread too thin. You'll have clear processes to follow and a strong partnership with your supervising PTs for backup and guidance. They also mention scheduling flexibility as one of the resources available to you.
