Case Manager RN

The Job

TriStar Southern Hills Medical Center is looking for a Case Manager RN to support patients and families through every stage of their care journey. This isn't just about checking boxes ; it's about coordinating services, linking people to the right resources, and building care plans that actually fit each patient's life. You'll work alongside physicians, nurses, and department leaders to make sure transitions are safe, communication is clear, and quality stays consistent from one end of the care continuum to the other.

Here's what you'd be doing day to day:

• Guiding patients and families through program orientation, explaining the rehab philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers so they can participate actively in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient's strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, documenting everything clearly and communicating needs across the care team
• Working closely with the Rehab Program Director, Facility Case Management Director, and the rest of the interdisciplinary team to deliver seamless, patient centered care

What You'll Need

• An active RN license
• An Associate Degree in Nursing or higher
• At least 3 years of nursing experience in an acute care setting
• Case Management experience is preferred
