What you’ll do as a Case Manager RN

You’ll handle patient assessments that cover medical history, medications, treatment plans, and psychosocial needs. It’s a deep look at the whole person, not just the diagnosis.

• Collaborate with doctors, nurses, patients, and families to build individualized care plans that actually work for each situation
• Track how patients are doing and adjust their care plans when something isn’t working ; then make sure the healthcare team knows about any changes
• Act as the main contact between patients and their providers so nothing gets lost in translation; communication stays clear and continuous
• Keep detailed records and case notes up to date, following all confidentiality and compliance rules without cutting corners

What you need to qualify

• A Bachelor of Science in Nursing (BSN) or higher from an accredited program
• An active and unencumbered Registered Nurse license
• At least one year of experience in case management ; ideally within a hospital or healthcare setting
• Solid interpersonal skills for working with medical pros, patients, and families (you’re good at explaining things clearly both in writing and conversation)
• A working comfort level with electronic health records (EHR) software
