What you'll do
Your day isn't in an office or a clinic. You're out in the community, traveling to where your patients live. You're their consistent point of contact, and that requires a unique blend of clinical skill and human connection.
- You monitor patient progress closely, tracking any changes in their health and reporting back to the supervising RN.
- The plan of care isn't static; you'll constantly evaluate what your patient needs and adjust your approach.
- A big part of the job is coordination. You'll communicate with our internal team, outside providers, and community resources to make sure care is seamless.
- Managing your weekly caseload means being organized and agile as you move between visits.
- You'll provide hands on nursing care within the LPN scope of practice.
- We equip you with technology; a tablet, phone; to keep patient records accurate and up to date right from the field.
- You’ll provide education and support to patients and their families, helping them navigate complex situations, including end of life care.
What we need from you
This isn't just another nursing job. It's autonomous work that demands solid experience and a real comfort with being on the road, connecting with people one on one.
- A current New York State LPN license is required.
- A high school diploma or completion of an LPN program is required.
- We're looking for someone with at least one year of experience as an LPN. Experience in home care or hospice is a definite plus.
- A valid driver's license may be needed depending on operational needs in Manhattan, NY.
- Bilingual skills might also be required for this area; it depends on the patients we serve.
