
About This Role
Here's the real deal on this hospice RN position with VNS Health in Manhattan, NY. It's not an office job. You'll be out in the community, providing the nursing care that lets people spend their final days at home, on their own terms. What you'll do
You're the primary nurse for your caseload of hospice patients. That means you own the relationship and the clinical plan. You'll travel to wherever your patients call home; apartments, houses, care facilities; to provide direct nursing care and support.
- Complete physical, psychosocial, and environmental assessments to understand the full picture of a patient's needs.
- Develop, implement, and constantly update a personalized nursing plan of care in partnership with the patient, their family, and our interdisciplinary team.
- Your visits are where the care happens. You'll manage symptoms, perform treatments, assist with daily living activities, and educate families on what to expect and how to provide comfort.
- You're the central communicator. You'll keep the physician and the rest of the care team informed of any changes in the patient's condition or needs.
- The work continues for the family after a loss. You'll provide bereavement support and connect them to helpful resources.
- This role involves teaching and supervising LPNs and home health aides to ensure consistent care between your visits.
- You'll participate in regular team meetings to coordinate care for every patient on our service.
A day on the road
Forget a desk. Your "office" changes every visit. You need to be ready for walk up buildings, different family dynamics, even pets. You'll carry a nursing bag with supplies that weighs about 25 30 lbs. The physical part is real; you're bending, moving, and assisting patients of all sizes. It's active work that directly translates to someone else's comfort.
What you need to bring
- A current New York State license as a Registered Professional Nurse.
- An Associate's Degree in Nursing plus at least one year of full time experience as a staff nurse in med/surg or another acute care area. A Bachelor's Degree in Nursing from a state approved program also meets the requirement.
- A minimum of one year of clinical nursing experience is required. At least six months of that should be in a relevant setting: Hospice, Med/Surg, ICU, Emergency Department, Home Care, Palliative Care, Oncology, or Long Term Care.
- You need reliable transportation to get to patient homes across your assigned area.
- Basic computer skills are needed for documentation and communication.
The details
The annual pay range for this position is $118,942 - $152,717.
