
About This Role
The Role
You will monitor and assess how home health field services are delivered. This means supervising Home Health Aides (HHAs) in the field, jumping into case management when it's needed, and providing clinical support. You'll help develop care plans and follow up on them. Think of it as the bridge between our clinical team and what actually happens in a patient's home.
What You'll Actually Do
- Go into patients' homes for regular supervisory visits ; assessing, training, coordinating care, and handling any crises that come up.
- If there are significant changes in a patient's condition, or if an emergency intervention or care plan change is needed, you'll communicate that clearly to the clinical manager and document everything electronically.
- Work with the primary care physician and the rest of the clinical team to complete a full patient assessment. You'll gather the patient's medical history and medication information.
- Supervise the initial home assessment to make sure the HHA is following VNS guidelines and regulations.
- Figure out where the Home Health Aide needs more training and document those needs.
- Deliver the start of care packet and review it with the patient or caregiver during your first visit.
- Educate the HHA about the paraprofessional plan of care and review it with them.
- Get involved in quality assurance and education programs when management asks you to.
- Take on special projects and any other duties that come up.
What You Need to Bring
- An active Registered Professional Nurse license in New York State. That's non negotiable.
- CPR/BLS certification. You'll need it current.
- An Associate's Degree in Nursing from an approved program is the minimum. A Bachelor's Degree in Nursing from an approved program is preferred.
- At least two years of clinical experience as a Registered Nurse. That's required.
- If you have home care experience in a licensed home care service agency or a CHHA setting, that's a strong plus.
Pay Range
$77,200.00 to $96,500.00 per year.
