The Role

This is a Case Manager position at Encompass Health Rehabilitation Hospital of Fitchburg in Madison, WI. You'll be the link between clinical care and the patient's journey, from admission all the way through to post discharge. • Work with the interdisciplinary team to guide treatment plans based on what each patient needs and prefers.

• Stay current on regulations, standards, company policies, procedures, and department operations.
• Review case management reports like Key Care Indicators and figure out what actions are needed.
• Understand commercial contract levels, exclusions, payor requirements, and recertification rules.
• Attend Acute Care Transfer (ACT) meetings to spot trends and work together to reduce those transfers.
• Meet with each patient or their family within 24 hours of admission per the Patient Arrival and Initial Visit Standard.
• Do a full assessment of goals within 48 hours of admission and complete that case management addendum.
• Educate patients and families about rehabilitation itself plus what your role as Case Manager means; set up a communication plan so everyone stays informed.
• Schedule family conferences when needed ; they could happen more than once depending on what's going on ; then facilitate those meetings yourself.
• Help patients get resources quickly enough so nothing causes delays or problems at discharge time.
