
About This Role
Alright, let's talk about this job. It's not your standard clinic gig. You'd be leading a team that goes into the community to care for our most complex patients, the ones at the highest risk of ending up back in the hospital. The goal is simple in concept but tough in practice: keep them healthy, stable, and at home.
What you'd be doing
You're the clinical lead for a Community Care team. That means RNs, LPNs, Social Workers, and Health Coordinators all look to you for direction on our toughest cases. Your main focus is on three groups: patients transitioning from a facility back to us, high risk patients who are struggling, and patients we've been assigned who aren't yet engaged with care at all.
- You'll do the initial in home or telephonic assessments for these high risk patients and design their comprehensive plan of care.
- A big part of your week involves providing clinical supervision to other team members as they carry out those plans.
- You'll conduct joint supervisory visits with LPNs and their patients to oversee care and handle discharges from case management.
- Your assessments cover everything from disease monitoring and meds to health education and screening for social needs like food or housing insecurity.
- Coordinating that plan of care is key. You make sure it reflects what the patient actually needs and what services are available to them.
- You're the connector; helping patients navigate the healthcare system, linking them with community resources, and orchestrating all the moving parts of their care delivery.
- The role involves maintaining ongoing communication with families and community providers to promote the patient's wellbeing.
- You'll monitor the quality of home health aide visits and other outpatient services.
- Sometimes you'll do urgent home visits under a physician's direction to meet a patient's immediate need.
