About This Role

Alright, let's talk about this PRN LPN role for home health in Kansas City, MO. It's not your typical clinic gig. You're going into people's homes, which is a different kind of nursing. You're there to help them manage, to teach, and to give them the support they need to stay as independent as possible. It's one patient, one family, at a time.

What you'll do

You'll be the nurse visiting patients where they live. Your day is built around those visits.

  • Handle direct nursing care during home visits. That means wound care, giving medications, and assisting with any needed procedures.
  • Keep a sharp eye on how your patients are doing. You're their advocate, so you'll note any changes and communicate them quickly to the RN or the rest of the care team.
  • Your documentation matters. It has to be clear, thorough, and accurate to keep care safe and consistent between visits.
  • A big part of the job is teaching patients and their families. You'll help them understand conditions, manage medications correctly, and learn how to handle things at home.
  • You don't work in a vacuum. You're collaborating constantly with RNs, doctors, therapists, and care coordinators to make sure the whole plan comes together for the patient.

What you need to have

This isn't an entry level position. They need someone who's been in the field and knows how to work autonomously.

  • A current LPN or LVN license is mandatory.
  • You have to be a graduate from an accredited vocational school for practical/vocational nursing.
  • They require at least one year of nursing experience.
  • A valid driver's license is non negotiable. You're driving to see your patients.
  • The schedule is PRN; that's "as needed" or intermittent. So availability for that kind of flexible scheduling is part of the deal.
