The Job in a Nutshell

You'd step in as a Regional Grant Coordinator supporting Indiana's 5 year, statewide Rural Health Transformation Program (RHTP). Specifically, you'd work on the Make Rural Indiana Healthy Again (MRIHA) Regional Grants. These are multi year investments aimed at strengthening rural healthcare access, prevention, and system resilience across the state. It's a full time, hybrid position.

Important: Only work authorizations that don't require sponsorship now or in the future will be considered. You need to live in or be willing to travel frequently through Knox, Daviess, Martin, Gibson, Pike, Dubois, Posey, Vanderburgh, Warrick, or Spencer counties. You might also need to head to Indianapolis now and then for mandatory in person meetings, but they reimburse you for that. The schedule is 37.5 hours per week.

What You'll Actually Do

  • Show up for regular check ins, statewide meetings, and required trainings
  • Serve as the primary regional contract manager for organizations getting RHTP funding
  • Keep an eye on compliance with state and federal requirements ; think allowable expenditures, budget adherence, and documentation standards
  • Help the Indiana Department of Health (IDOH) make sure regional coalitions hit their milestones, deliverables, and timelines from the state defined workplan
  • Spot risks, performance issues, or compliance concerns and flag them to IDOH
  • Maintain audit ready documentation and consistently use state approved templates, tools, and reporting systems
  • Work closely with statewide Technical Assistance Providers like IHA, IPHCA, and IRHA to keep support aligned for regional coalitions
  • Join regional meetings run by Technical Assistance Providers, including consortium meetings, workgroups, and application development sessions
  • Help regions learn from each other by sharing insights, challenges, and best practices
  • Coordinate regional data collection, performance tracking, and outcome measurement tied to CMS Rural Health Transformation Program requirements
  • Ensure all required regional reports get submitted to IDOH on time for state and federal reporting cycles
  • Support regional partners as they figure out CMS reporting expectations ; documentation, metrics, narrative requirements, the whole deal
  • Act as the day to day liaison between IDOH and regional coalitions, which includes hospitals, clinics, EMS providers, community organizations, and others
  • Promote broad representation of rural populations and make sure regional activities actually reflect local needs and priorities
  • Keep communication channels strong with IDOH's Regional Grant Initiative Team and show up to statewide meetings as requested
  • Help regional partners interpret state guidance, funding requirements, and program expectations
  • Track regional progress, identify barriers, and support problem solving to keep momentum going
  • Contribute to statewide consistency by using shared tools, templates, and evaluation frameworks

What You Bring

You need a Bachelor's degree in public health, health administration, public policy, business administration, or something closely related. On top of that, you've got 5 years of experience in grant management, contract oversight, program administration, or similar work. You also have 3 years of experience working with healthcare organizations, rural communities, or public health systems. And you've got 2 years of experience dealing with compliance, budgeting, and reporting requirements for state or federal grants.

You can manage multiple projects at once without losing the details. Your communication, facilitation, and relationship building skills are solid. You're comfortable working independently as a remote field based contractor. You must live within the assigned RHTP region and be willing to travel regularly within it.

It helps if you have experience with CMS funded programs or federal cooperative agreements. Familiarity with Indiana's rural health landscape ; including hospitals, FQHCs, EMS, and community based organizations ; is also a plus. Same goes for experience collaborating with Technical Assistance Providers.
