The Role
ChenMed is looking for a Pharmacist in Houston, TX. This isn't a typical retail gig. You'll work embedded in a primary care clinic, under a collaborative practice agreement. Your focus is on the patients ; cutting down on unnecessary meds, getting adherence up, and managing chronic conditions like diabetes and hypertension. You're part of a care team that includes physicians, nurses, and advanced practice providers, and your work directly impacts quality metrics like CMS Star Ratings and HEDIS scores.
What You'll Do
- Work with physicians and providers to adjust medication therapies, interpret lab results, and support clinical decisions within established protocols.
- Conduct comprehensive medication reviews ; looking at safety, efficacy, cost, side effects, and barriers to adherence for each patient.
- Proactively manage chronic diseases like diabetes, hypertension, anticoagulation, and asthma to prevent complications.
- Smoother transitions of care by resolving medication discrepancies when patients leave the hospital and helping prevent readmissions.
- Counsel patients one on one about their medications, side effects, and strategies to stick with their regimens.
- Order and interpret lab work to monitor how medications are working and whether they're safe.
- Help implement care protocols alongside physicians, including making independent medication adjustments where the agreement allows.
- Answer physician consults, provide drug information, and support quality improvement projects. Other duties as assigned by your manager.
What You Need
- A Doctorate of Pharmacy (PharmD) from an accredited program.
- A valid pharmacy license in good standing.
- At least two years of clinical experience in primary care, ambulatory care, managed care, or a related setting.
- Solid knowledge of state and federal insurance programs (Medicare, Medicaid) and pharmacy regulations.
- Comfort working both independently and as part of a multidisciplinary team.
- Strong organizational and clinical decision making skills, plus attention to detail.
- Willingness to travel locally up to 50% of the time. Also need flexibility to work evenings, weekends, or holidays when needed.
- Fluency in spoken and written English.
Nice to Have
- Pharmacy residency or ambulatory care certification (like BCACP) ; highly preferred.
- Immunization certification and experience with medication therapy management.
- Experience in managed care, HMO, PBM, or MSO settings.
Pay & Benefits
The base salary for this role in Houston, TX ranges from $118,062 to $168,658 annually. Your actual pay depends on experience, education, geographic location, and other factors. This position may also be eligible for bonuses or commissions.
