About This Role
Alright, here's the deal. We need a weekend warrior of an RN Case Manager at HCA Houston Healthcare Kingwood in Houston, TX. This isn't your standard Monday to Friday gig; it's built for someone who thrives on a different rhythm and wants their weekdays free.
The Schedule (Pick Your Flavor)
You've got two solid options. You can work four 10 hour shifts: Friday, Saturday, Sunday, and Monday. Or, you can go for three 12 hour shifts: Saturday, Sunday, and Monday. It's full time hours packed into the weekend.
What You'll Be Doing
You're the central point for making sure a patient's hospital stay is appropriate and that their transition out is smooth and safe. You'll review cases to ensure we're using resources wisely and that the care plan makes sense from admission through to discharge planning. A big part of your focus is on preventing avoidable readmissions by nailing down what a patient needs after they leave us.
- Review patient stays for medical necessity and appropriate length of stay.
- Coordinate services and resources throughout the entire care continuum.
- Assess patients for discharge needs to ensure a safe transition out of the hospital.
- Your scope covers everyone; from newborns to geriatric patients; so you need to be comfortable across all age groups.
What You Need to Bring
- A current Texas RN license from the State Board of Nurse Examiners.
- An Associate Degree in Nursing is required; a Bachelor's Degree is preferred.
- At least one year of experience working in healthcare.
- A Case Management certification would be great to see, but it's not a must have starting out.
