Alright, let's talk about this Home Therapy RN role in Honolulu, HI. This isn't just another nursing job. You're the coordinator, the teacher, and the advocate for patients dialyzing at home. You'll be the one making sure they can do this safely and independently. That means getting out of the clinic and into their homes to assess the environment, ensuring everything meets CMS standards. It's a different kind of nursing, built on deep relationships and a holistic view of care.
What you'll do
You're the central point for a patient's home dialysis care. You'll collaborate with the full health team to make it all work.
- Act as a teacher and an advocate for your patients, helping them understand their treatment and make informed decisions.
- The goal is always to support their maximum desired independence.
- You'll conduct home visits. A big part of your job is evaluating whether a patient's home is suitable and safe for dialysis, following all CMS regulations.
- Professional growth matters here. You'll be involved in continuing education, quality improvement projects, and applying new research findings to your practice.
- You’ll navigate ethical challenges and sound practices, working with mentors when you need to.
What you need to bring
The basics are non negotiable. We also need someone who sees this as more than a task list.
- A current, active, and unencumbered RN license for the state.
- Graduation from an accredited nursing school or equivalent program.
- At least one year of experience working as a registered nurse.
- Specifically, you need one year of experience as a Nephrology Nurse with dialysis experience. That experience must be in peritoneal dialysis, home hemodialysis, or a combination of both.
- A current Healthcare Provider CPR certification is required.
- A valid state driver's license and reliable personal transportation are mandatory for those home visits.
- You should be comfortable using Electronic Health Records; competent, in depth knowledge is key.
- The ability to organize tasks, prioritize what's important, and see projects through on your own initiative is essential.
