
About This Role
The Role
Hartford Hospital, one of Connecticut’s largest teaching hospitals and a Level I trauma center, is looking for a Social Worker to join its Transplant team in Hartford, CT. This isn’t a generalist position ; it’s a specialized role focused entirely on transplant patients from the moment they’re referred, through their hospitalization, and for the entire post transplant period. You’ll provide pre transplant education and emotional support, coordinate care with other team members, connect patients to community agencies, and help with concrete services. Every patient you evaluate gets presented at a multidisciplinary team meeting where all disciplines participate.
Schedule
Monday through Friday, 8:00 AM to 4:30 PM.
What You’ll Need
- A master’s degree in social work.
- Active membership in good standing with STSW.
- Certified Clinical Transplant Social Worker (CCTSW) credential ; you need to earn this within three years of joining the team.
- An LCSW license.
- At least two years of social work experience in a hospital or healthcare setting.
- Transplant social work experience is preferred, not required.
- Strong interpersonal and written communication skills.
- Current clinical assessment skills.
- The ability to work autonomously and direct your own work.
