About the Job

This is a Nurse Practitioner role with Upward Health. We're a medical group that provides care in patients' homes, treating everything from physical to social health needs. Your work will be mostly virtual, using our own platform to connect with patients who have complex, chronic conditions. The goal is straightforward: improve outcomes and reduce costs by meeting people where they are.

You'll also step out for in person visits when it's needed. That could be for an annual wellness check, to help someone transition from a hospital stay, or for an urgent primary care need. You're the day to day clinical lead for a multidisciplinary team, making decisions under the guidance of our Medical Director.

What You'll Do

  • Triage, diagnose, and treat patients, both virtually and in person.
  • Act as the clinical leader for your team; you'll provide decision support and direction.
  • Handle primary care visits, urgent issues, and annual wellness exams. You'll make sure all the right screenings and quality measures are covered.
  • The job involves referring patients out for behavioral health or specialty services when necessary.
  • A big part of this is coordinating care to prevent people from going back to the hospital. You'll collaborate with primary care doctors, hospitalists, and nursing facility providers.
  • You're on the rotating on call schedule for our 24/7 patient hotline. That means some nights and weekends.
  • Work closely with the entire interdisciplinary team. You'll join daily huddles and patient discussions.
  • Keep an eye on patient data that comes in remotely from monitoring devices and respond to it.

What We're Looking For

We need someone who knows how to manage complicated cases. You should be comfortable assessing and treating high acuity patients with multiple chronic conditions. Experience with Medicare and Medicaid populations is important, whether you've worked inpatient, outpatient, or in home care.

  • A minimum of five years working as a provider. At least three of those should be in a primary care or closely related setting.
  • You must have a multi state Advanced Nursing License or be willing to get one promptly.
  • A valid driver's license and auto liability insurance are required.
  • While most work is virtual, you have to be willing to travel locally around Hartford, CT when an in person visit is called for.

If you have skills in motivational interviewing or trauma informed care, that's a definite plus. So is any background in an urgent or emergent care setting. But more than anything, we need someone dedicated to health equity who wants to transform how care is delivered at home.

The Practical Details

The pay range for this position in New York is $135,000 annually.
