What you'll do
The work is all about patient care and coordination. You'll evaluate and treat speech, voice, language, hearing, and swallowing disorders. That means creating plans, running diagnostic tests, and applying therapeutic treatments.
- Develop and review patient Plans of Treatment and Care.
- Your observations on a patient's condition and response to treatment are critical; you'll report those changes to supervisors and physicians.
- There's a teaching component. You'll instruct patients on using alternative communication systems and counsel patients and their families.
- A big part of the role is documentation; timely written reports of evaluations, visits, summaries, and progress notes are part of the deal.
- You'll participate in care coordination activities and discharge planning.
- Confidentiality is non negotiable. You maintain the highest standards with patient information.
- Continuing education is expected; you'll attend internal development programs as required by company policy.
What you need to bring
The requirements are specific. We need someone who's properly credentialed and ready for the autonomy of home health.
- You must meet the education and experience requirements for the Certificate of Clinical Competence in Speech Language Pathology (CCC SLP) from ASHA.
- A minimum of six months working as a speech therapist or pathologist is required.
- Having some home health experience is a definite plus.
- A current, unrestricted state license to practice is mandatory.
- You need current CPR certification.
- A valid driver’s license, auto insurance, and reliable transportation aren't just nice to haves; they're required for getting to patient visits.

Pay & schedule
