Physical Therapist

Alright, let's talk about this Physical Therapist role at BoldAge PACE in Fresno, CA. It's not your typical clinic gig. You're embedded in an interdisciplinary team that manages the whole care plan for seniors who want to live at home. The work is split between our center, participants' homes, and sometimes contracted facilities. Your assessments and recommendations directly shape their daily lives.

What you'll do

You're the expert on movement, safety, and function for our participants. A big part of your week is spent in our daily team meetings, collaborating on care plans.

• Conduct physical therapy assessments when someone new joins us and whenever the care team decides it's needed.
• Figure out what durable medical equipment, special shoes, or prosthetics a person needs.
• Go into participants' homes to evaluate safety and mobility; you'll recommend modifications and equipment.
• Develop and carry out treatment plans that meet professional standards and our own policies.
• Your documentation has to be spot on and timely; evaluations, plans, treatments, all of it.
• Teach participants and their families how to do home programs; you need to document that they understand.
• Train our staff on things like safe body mechanics, using equipment, transfers, and helping with daily activities.
• You'll step in to cover duties for other team members if your license and skills allow it.
• Participate in quality improvement projects and a variety of other meetings related to care or operations.
• Follow all our policies, procedures, OSHA guidelines, and confidentiality rules to the letter.

The background we're looking for

The non negotiables are clear. We also have some strong preferences that would make someone a great fit from day one.

• A current license to practice Physical Therapy in this state is mandatory.
• You need an advanced degree in PT from an accredited school.
• A driver's license and proof of auto insurance are required for travel to homes and facilities.
• It'd be fantastic if you're bilingual and fluent in Hmong; that's a real asset here.
• We'd prefer two years of experience working on an interdisciplinary team in a hospital, nursing home, or community setting.
• A year of experience with a frail or elderly population is also preferred. If you don't have that specific experience yet, we'll provide training.

The schedule & pay

This is a full time position. The starting pay range is $45-$55 per hour. There's also a $5,000 sign on bonus attached to this role.
