What This Role Is About
BoldAge PACE runs an all inclusive program of care for older adults. We tailor everything to each person's health and well being needs. Our approach is simple: we listen to our participants and their caregivers so we actually understand what they need and want.
As a Certified Occupational Therapy Assistant, you'll work under the direct supervision of an Occupational Therapist. You'll provide care that helps people stay independent, safe, and functional. You'll implement therapeutic activities and interventions based on individualized treatment plans, supporting each participant's physical, cognitive, and psychosocial needs.
What You'll Actually Do
- Help participants with activities of daily living like grooming, dressing, and meal preparation ; always focused on their independence, dignity, and safety
- Deliver therapeutic interventions under OT supervision; use adaptive equipment and techniques to support functional improvement
- Carry out individualized treatment plans, adapting techniques as participants progress or their needs change
- Use creative solutions to keep participants engaged and moving toward therapy goals
- Make sure transfers, ambulation, and mobility are safe; teach participants and caregivers how to use assistive devices properly
- Observe participant progress carefully; document changes accurately in the EMR; report concerns promptly to the OT or interdisciplinary team
- Work closely with the interdisciplinary team (IDT) to provide holistic care that centers on each participant
- Attend IDT meetings regularly; share updates and insights from your work with participants
- Support both group and individual activities that promote physical, cognitive, and emotional well being
- Take part in quality assurance/quality improvement initiatives as part of the overall quality program
- Follow BoldAge PACE policies, safety protocols, ethical guidelines; maintain HIPAA and OSHA compliance at all times
What You Need to Bring
- Bilingual fluency in Hmong is required.
- A completed Associate Degree from an accredited Occupational Therapy Assistant program is required.
- A passing score on the National Board for Certification in Occupational Therapy (NBCOT) exam ; earning you the title COTA.
- State licensure as required by regulations where you practice.
- Completion of Level I and Level II Fieldwork in various care settings like rehab centers, schools, hospitals, or skilled nursing facilities.
- A minimum of 1 year working with elderly populations is required. If you don't have that yet but are otherwise qualified? Training will be provided upon hire if applicable for this role.
- (Preferred): Experience in long term care, hospice, or rehabilitation settings helps but isn't mandatory.
The Pre Employment Stuff (Straight Talk)
You'll need reliable transportation with a valid driver's license plus at least the minimum state required liability auto insurance. Before any direct participant contact you must be medically cleared for communicable diseases with all immunizations up to date. And we run a comprehensive background check including federal/state Medicare/Medicaid exclusion lists criminal history education verification license verification reference checks plus a drug screen. It's thorough but it matters for this work.
