Nurse Practitioner

Kaiser Permanente·Fresno, CA

About This Role

What you'll actually do

You'll assess patients in the designated service ; that's the core of this Nurse Practitioner role at Kaiser Permanente in Fresno, CA. You'll do physical exams, talk with patients in person and over the phone or electronically, and work with others in the department to keep patient care flowing smoothly. You're reporting to both the Chief of the service and the Department Manager.

You'll handle age appropriate assessments, take histories, and do physicals that match department protocol. The assessment part is ongoing and systematic ; you'll look at physiologic, psychosocial, cultural, spiritual, and cognitive status, plus patient education, family involvement, and advocacy.

You'll consult with the supervising physician or other clinicians when the patient's needs call for it. And you'll work alongside physicians and other clinicians to keep patient care consistent with quality indicators for the service. You'll review medical records for follow up or ordering diagnostic studies under established protocols.

Communication matters here. You'll stay in touch with other providers and departments to keep operations efficient and care consistent. You'll build working relationships with the health care team, patients, and families. You'll acknowledge staff rights and cultural and ethical beliefs. You'll delegate appropriately and coordinate duties for the team, then evaluate how effective the care actually is.

You're expected to know the institution's safety principles and apply them. When emergencies happen, you'll follow protocols, stay calm, inform the right people, and document everything. You're responsible for your own professional practice and you'll participate in staff development activities for your unit and department. You'll know the legal stuff ; patient confidentiality and risk management ; in every aspect of care and unit functioning. You'll join in Performance Improvement activities, and nursing research when asked. You'll identify problems and solve them. You'll approach every interaction with patients, families, and team members with a service philosophy. And you'll be proficient with computers and telecommunication tools for documenting, tracking, and conveying info. If you're in a specialty role, you'll meet the expectations for that role. Other duties come up as needed.
