What This Role Is About

This is a PRN (as needed) Registered Nurse position with Discovery At Home Texas in Fort Worth, TX. You'll provide skilled nursing care to patients in their own homes. You're not on a set schedule; you get called in when the organization needs you. The core of this job is working with an interdisciplinary team, physicians, and families to build a care plan that actually fits what the patient needs.

What You'll Actually Do

Assess the patient's condition and collaborate with the team to implement the care plan
Provide wound care, medication management, IV therapy, and other skilled nursing services
Keep the patient and their caregiver educated on managing the disease, preventing complications, and self care
Work closely with physicians and other healthcare pros to develop and adjust the plan of care
Complete patient documentation accurately and on time
Take part in quality assurance and performance improvement activities
Attend interdisciplinary team meetings when needed

What You Need to Bring

An active Texas RN license
At least 1 year of nursing experience; home health experience is preferred
The ability to work independently with very little supervision
Strong clinical and critical thinking skills
Good communication skills and the ability to connect with people
Reliable transportation to get to patients' homes
Willingness to work on an as needed basis

The Physical Side of the Job

You'll be using your arms, legs, and whole body a lot ; lifting, balancing, walking, stooping, and handling materials. You'll also need to navigate different home environments and adapt to both indoor and outdoor conditions. It's not a desk job.

Schedule

This is a PRN (as needed) position. You get called in when the organization has a need. There's no fixed weekly schedule.
