About This Role

Here's the thing about being a Case Manager RN at Medical City Dallas: you're the one who makes sure the pieces fit. You're not just managing a case; you're building a bridge for someone from hospital to home, and you've got to get it right.

The Work

Your day is about connection and coordination. You'll start with a deep dive, assessing everything from medical needs to what kind of support system a patient has waiting for them. Then, you build the plan.

  • You coordinate that plan of care and drive the discharge process by working directly with doctors, therapists, social workers; the whole team.
  • Reassessing patients as things change is part of the job; their condition isn't static, and your plan can't be either.
  • You'll partner closely with Social Services. If there's a risk related to things like housing or food access, you get them involved. For post acute medical needs, you work together to make sure the next level of care is appropriate.
  • A big part of this is keeping an eye on length of stay and resource use. You'll use tools like InterQual criteria to evaluate how care is progressing and flag any issues early.
  • Patients and their families are part of your team. You bring them into the conversation to set clear, realistic goals together.
  • If someone's at high risk for readmission, you develop strategies to prevent it. That could mean education on using healthcare resources properly or connecting them with community support.
  • You'll also handle referrals to third party payers and disease management programs when needed.

The Schedule & Details

This is a full time position on the day shift. Your hours will typically be 8:00 AM to 4:30 PM, Monday through Friday. And yes, there will be some weekend shifts involved.

A sign on bonus up to $20K may be available for new hires.

What You Need To Bring

  • A current RN license in Texas or a compact state license is required.
  • You need an Associate's Degree in Nursing (ADN) as a minimum; a Bachelor's Degree in Nursing (BSN) is preferred.
  • Candidates must have at least three years of recent clinical nursing experience in an acute care hospital setting.
