• Complete psychosocial assessments and discharge planning evaluations. You've got to document your findings clearly and communicate those needs to everyone on the care team.
• Work hand in hand with the Rehab Program Director, the Facility Case Management Director, and the entire interdisciplinary team to make sure care is seamless and centered on the patient.

The background we need

We're looking for a specific mix of education and hands on experience.

• You need an Associate Degree in Nursing or a Nursing Diploma. A Bachelor’s Degree in Nursing would be a plus.
• For experience: either 2+ years specifically in case management or 3+ years in clinical nursing is required.
• If you've worked with InterQual before, that's preferred but not a deal breaker.

Licenses & certifications

• A current Registered Nurse license for the state where you'll practice is mandatory.
• A Certification in Case Management is looked on favorably.
