
About This Role
Alright, let's talk about this Case Manager RN role at Del Sol Medical Center in El Paso, TX. This isn't just a paperwork job. What you'll be doing
You'll manage the patient's plan of care for their whole stay. That means you're steering the ship on length of stay, making sure resources are used appropriately, and building a solid discharge plan with the whole team. You'll use established criteria to move care forward smoothly.
The goal is quality outcomes and keeping patient flow moving, all while balancing great care with smart resource use.
- Walk patients and their families through how things work here. You'll explain our rehab philosophy, break down Medicare and insurance benefits, talk about discharge criteria, and review patient rights.
- Coordinate education so patients, families, and caregivers can really be part of the process. That includes treatment planning, goal discussions, and family conferences.
- Build and put into action individualized treatment plans. These plans should reflect what the patient is good at, what they need, and their personal recovery goals.
- Complete psychosocial assessments and discharge planning evaluations. You've got to document your findings clearly and communicate those needs to everyone on the care team.
- Work hand in hand with the Rehab Program Director, the Facility Case Management Director, and the entire interdisciplinary team to make sure care is seamless and centered on the patient.
The background we need
We're looking for a specific mix of education and hands on experience.
- You need an Associate Degree in Nursing or a Nursing Diploma. A Bachelor’s Degree in Nursing would be a plus.
- For experience: either 2+ years specifically in case management or 3+ years in clinical nursing is required.
- If you've worked with InterQual before, that's preferred but not a deal breaker.
Licenses & certifications
- A current Registered Nurse license for the state where you'll practice is mandatory.
- A Certification in Case Management is looked on favorably.
