About This Role

The Short Version

PruittHealth @ Home needs a Physical Therapist in Durham, NC for a full time home health role. This position comes with a $15,000 sign on bonus and uses a per point compensation model that pays more for visits requiring greater effort. It's top of market for the industry.

What You'll Actually Do

You'll work in patients' homes, which is where the heart of this job lives. Your day to day involves evaluating each patient's level of function and figuring out what skilled therapy they need. Then you treat them to relieve pain, restore function, and get the best possible outcomes based on the physician's plan of care.

  • Assess, monitor, document, and report patient progress using the company's preferred technology and within set timelines
  • Advise families and the interdisciplinary team on how to promote optimal functional outcomes
  • Collaborate with the attending physician and the team to develop, integrate, and manage the care plan, referring to other disciplines when needed
  • Conduct field visits for start of care evaluations and for clinical outcomes monitoring, follow up, and performance improvement
  • Use the company's teaching materials when educating patients and families
  • Complete documentation at the point of care and on time, including OASIS assessments, SOC/admissions, recertifications, ABNs, subsequent visits, physician orders, and care coordination
  • Meet agency productivity standards and use your time and resources well
  • Participate in center/agency surveys (Licensure and Joint Commission), mandatory in services, case conferences, staff meetings, and programs like the Performance Improvement Program
  • Follow safety regulations and report any job related hazards
  • Keep your license, certification, and clinical records current to meet federal, state, and agency rules
  • Recognize and respond to patient care priorities with initiative and sound judgment
  • Adapt to changes in workload and help train or orient others
  • Report any suspected violations to a supervisor or others until the issue is resolved
  • Work independently, prioritize your time, and hit deadlines for outcomes and strategic goals

What You Need

  • A valid professional license in the state where you'll practice
  • A degree from an accredited Physical Therapy program
  • If you're foreign trained, your program must be deemed substantially equivalent to a U.S. entry level education by a credentials evaluation organization approved by the APTA
