Case Manager RN

What you'll do

You'll guide patients and their families through the post acute care journey. That means coordinating care, communicating with providers, and making sure nothing falls through the cracks. It's a mix of clinical judgment, problem solving, and real human connection ; every day looks different.

• Coordinate patient care across settings, from hospital to home
• Work closely with physicians, therapists, and other nurses to keep care plans on track
• Advocate for patients and families, answering questions and easing transitions
• Document accurately and thoroughly in the medical record
• Collaborate with the care team to identify needs and adjust plans as things change
• Support discharge planning and follow up to reduce readmissions

What you need

• Active, current, unrestricted Registered Nurse (RN) license in the state where you'll practice
• 3 years of professional nursing experience
• A commitment to treating patients, families, and coworkers with respect and professionalism
• You're proactive, you collaborate well, and you take ownership of your work
