What you'll do
This is a job about managing the journey, not just the chart. You're the guide for patients moving from our centers into hospitals, rehab facilities, and back home again. Your main goal is to make sure nothing falls through the cracks and that patient outcomes are positive.
You'll manage care transitions and discharge planning for patients admitted to specific high priority hospitals. That means building real trust with patients and their families from the start. You introduce yourself, explain your role, and become their point person for questions and guidance.
- Collaborate with our clinical staff to develop and execute care plans.
- Coordinate everything with hospital case management departments and other facilities.
- Work with primary care physicians, hospitalists, specialists, and insurance case managers to transition patients to the right next level of care.
- Keep the primary care physician updated on a patient's status using email or our internal systems.
- Facilitate family conferences to review goals and plan for post hospital needs.
- Serve as the patient's advocate in every setting.
- Address advanced care planning topics like treatment goals and directives.
- Conduct concurrent medical record reviews using approved criteria.
- Promote effective use of clinical resources to achieve desired outcomes within specific timeframes.
The practical stuff
A lot of this role is about connection and communication. You're building bridges between a lot of different people and places.
- Establish relationships with patients' families, caregivers, social workers, other nurses, acute and post acute facilities, home health companies, and health plans.
- Build direct relationships with preferred hospitalists and specialists at key facilities.
- Refer complex cases involving psychosocial or economic needs to our social workers.
- Report any observed or suspected abuse as mandated by law.
The paperwork & compliance side
The advocacy work has an administrative backbone. It's how we ensure quality stays high across the board.
- Maintain detailed clinical notes for every patient. - Submit all required documentation on time in the appropriate systems. - Adhere strictly to departmental goals, performance standards, regulatory rules, quality measures, and company policies. - Participate in any assigned surveys or special projects. - Evaluate patient satisfaction levels. - Assist physicians in managing appropriate costs while achieving desired outcomes.A bit about us

What you need to bring
First off: an active Registered Nurse (RN) license in good standing for Detroit, MI. That's non negotiable.

- A Bachelor’s degree in Nursing (BSN).
- At least three years of experience working as an RN.
- At least one year of hands on experience specifically in Case Management or Utilization Review.
- Solid computer skills; you'll be navigating electronic medical records daily.

Schedule & Pay
This is a full time position working Monday through Friday during standard business hours. The pay range is $70k - $90k per year based on your qualifications.
