About This Role
Alright, let's talk about this Case Manager RN position at Presbyterian St. Luke's in Denver, CO. I've been placing nurses here for years, and their case management team is solid. They're looking for someone who gets the big picture of a patient's stay, from admission to getting them home safely.
The Work
You're the point person for coordinating the patient's entire plan of care during their hospital stay. That means you're managing the length of stay, making sure resources are used appropriately, and you're leading the charge on building a safe discharge plan with the whole care team. Your goal is to keep things moving smoothly toward quality outcomes; great care that's also efficient.
You'll facilitate care progression using established criteria, working alongside physicians, therapists, social workers; you know the drill. It's about making connections and navigating the system so nothing falls through the cracks for the patient.
What You Need to Bring
- A current Registered Nurse (RN) license.
- Either two full years of experience specifically in case management, or three plus years in clinical nursing.
- A Bachelor’s Degree is required.
This role is on the adult Med Surg units. The schedule is Monday through Friday, but there's a rotation; you'll cover one weekend per month.
The Pay & The Schedule
The hourly wage range for this role is $40.15 to $58.58. Just so we're clear: that posted range is what new hires typically fall into. Where you land depends on your specific skills and how your experience stacks up. Most folks are hired below the midpoint of that range.
Again, it's a day shift, Monday Friday job with that one weekend rotation each month.
