About This Role
The Job
This is a Clinical Case Manager position with WellPower in Denver, CO. You would work on our High Intensity Treatment Team (HITT), where you'll be the main point of contact for about 15 people on your caseload (caseload size changes as agency needs do). The team includes nurse practitioners, psychiatrists, nurses, and other case managers. It's primarily community based work ; you help people attend to their needs in their natural environments.
What You'll Actually Do
- Help the people on your caseload meet basic needs like food, clothing, shelter, personal safety, and medical and dental care
- Assist them with applications for income, medical, housing, or other benefits they're entitled to
- Identify when someone needs a protective payee ship and arrange that assistance
- Arrange transportation to services and community resources ; or transport people in your own vehicle (WellPower reimburses the additional business insurance cost)
- Provide comprehensive psychosocial services at high and low intensity levels to help improve or maintain each person's ability to function in normal social roles
- Identify strengths and symptoms with individuals to help set goals and connect them to appropriate resources
- Educate and support progress in daily living skills: medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, using transportation, other community services
- Help people develop social skills, interests, and leisure time activities ; including age appropriate and culturally appropriate daytime and evening activities
- Support individuals finding and connecting to employment opportunities that fit their recovery path
- Handle all clinical documentation of consumer interactions; assess monitor follow up on each person's ability to meet basic needs as they move toward recovery
Compensation & Benefits
Bachelor's degree level (CMI): $30.12/hour ; $62,650 annually. Master's degree level (CMII): $31.25/hour ; $65,000 annually. Language differential: An extra $1.00 to $2.50 per hour if you are bilingual proficient in Spanish or ASL.
