Hospice RN

The Role

As a Hospice RN Case Manager in Dayton, OH, you're the person who makes sure the care plan actually works. You'll oversee everything from the initial assessment through ongoing adjustments, working alongside a spiritual care coordinator, social worker, hospice aide, and medical director. Your job is to keep the whole picture in focus ; clinical, emotional, practical ; and make sure the patient gets what they need, when they need it.

You'll also communicate changes in patient conditions to the right providers and adjust the care plan as things evolve. It's hands on, collaborative, and you're never working in a silo.

What You'll Do

• Oversee how the patient's plan of care is implemented and reassess it regularly
• Work with the care team to develop a plan that actually addresses what the patient needs
• Keep providers informed of changing conditions so care stays coordinated
• Deliver one on one patient care that honors each person's dignity

Schedule & Flexibility

This is a full time role, Monday through Friday from 8:30am to 5pm. It also requires you to participate in a rotating on call schedule ; that covers evenings and holidays, so hospice patients and their families have support when they need it. What You'll Need

• An Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) is required; a BSN is preferred
• A current Registered Nurse (RN) license in the state where this position is located
• Current CPR and First Aid certification
• One year of professional RN experience in a hospice or similar setting ; that's required
• Reliable transportation to get to and from patient homes and work locations
