What you'd be doing

You'd be working as part of an interdisciplinary hospice team. Your core focus is understanding the social, spiritual, and emotional factors affecting a patient's health to help build a care plan that truly honors their dignity.

  • Assess the patient's and their chosen family's capacity to cope with daily living and a terminal illness.
  • Help prepare patients and families for changes, which includes education on advance directives and care planning.
  • Connect people with all available resources, from family support to community agencies.
  • Participate in developing and reviewing care plans for every patient on your caseload.
  • A big part of this role involves discharge planning when a patient's level of care or location needs to change.
  • You'd also provide support for the program's bereavement services.

The background we need

This isn't an entry level gig. You need solid professional experience post grad.

  • A Master’s Degree from an accredited school of Social Work or a closely related field is mandatory.
  • You must have at least one year of professional social work experience in a healthcare setting after getting your degree. Two or more years is preferred.
  • A valid state license or permit to practice clinical social work is required if our agency operation mandates it for the role.
  • A background in Medicare, Medicaid, or insurance specialty work is a plus.
  • Any experience or education in grief counseling would be really valuable here.

  • You need to be a licensed driver with reliable transportation to get to our business locations as needed.
