Licensed Vocational Nurse (LPN) – OneHome

This is a role for someone who takes their nursing license seriously and wants to work with a team that coordinates care for people in their own homes. OneHome is part of Humana, and we’re looking for an LPN to work a variable schedule in Corpus Christi, TX. You'll be providing nursing services based on each patient's plan of care, and you'll report any problems or potential issues to the RN or Case Manager.

What you’ll actually be doing

• You’ll give input to the client’s plan of care as part of the nursing process
• Implement the care plan exactly as it's written
• Evaluate how effective the care plan is and recommend revisions when it makes sense
• Record all observations and treatments per Agency policies
• Handle the emotional and physical comfort and safety of each client
• Perform nursing procedures you’ve been trained on
• Notify the right person about any unusual symptoms or changes in a client’s condition
• Participate in case conferences, meetings, and performance improvement activities as assigned
• Provide proof of a current Texas license and verify your education and work history
• Practice safely and competently within your job description
• Comply with all state regulatory acts and Agency policies, including HIPAA Privacy Rules
• Provide documentation for high tech certification if it applies; only perform duties you’re qualified for
• Maintain patient and business confidentiality

What you’ll need to bring

• You’re a graduate of an approved school of nursing and currently licensed in Texas
• At least one year of current experience in an accredited hospital or accredited nursing home within the last two years
• You’ve got excellent interpersonal and organizational skills
• You can work independently without someone looking over your shoulder

Schedule and pay

This position has a scheduled weekly hours of 1, so it’s a variable schedule. The pay range for full time (40 hours per week) starts at $53,000 and goes up to $73,000 per year. That’s a good faith estimate based on location and your experience, education, and certifications at the time of posting.
